The form of confirmation are the words that the minister says which are "Be sealed with the gifts of the Holy Spirit".

Holy Chrism, which is a perfumed oil, and the laying on of hands by the minister is the matter of the sacrament of Confirmation.
Who are the Ministers?
The ordinary Minister of Confirmation is the Bishop, which are successors of the apostles. The Extraordinary Ministers of Confirmation is a priest with permission from the bishop. A pastor of a parish can also confirm the people entering the church the night of the Easter Vigil.
Who can receive Confirmation?
People who have been baptized and are in need of completing their baptismal graces. The Church doesn't recognize confirmation on other faiths, so if someone converts they must receive Confirmation in the Catholic Church. You can only receive confirmation ONCE because it leaves a permanent mark on the soul much like baptism.

Confirmation is a sign of consecration, it leaves an indelible spiritual mark on the soul, completes the Baptismal grace, and we receive the seven gifts of the Holy Spirit first given to us at Baptism. The seven gifts of the Holy Spirit are wisdom, understanding, counsel, fortitude, knowledge, piety, and fear of the lord.
Why I chose my Confirmation Name?
I chose the name Leonard after two different people. The first was St. Leonard of Noblac. St. Leonard was a great hero who freed people from prison and saved many lives. He is definitely an unknown hero who put other peoples lives in front of his own. The other person is my grandpa Leonard. He was always there for me when I needed him most. He was a great person to follow and I hope I can live a life similar to his.

He is a great role model who put other people before himself similar to St. Leonard of Noblac. My Grandpa always looked at the bright side of things and that is why I wanted him to be my sponsor.
